POSITION:
Short Term Urgent Response-Social Services, Catulpa Community Support Services, Barrie with responsibility throughout Simcoe County- Hybrid Work Model
PURPOSE OF POSITION:
The provincial Urgent Response process exists alongside other developmental service mechanisms to respond to adults with a developmental disability who are experiencing an unmet urgent developmental support need(s). The role of Short-Term Urgent Response is to work in collaboration with the Crisis Response Network Coordinator and Urgent Response Case Manager, in compliance with the Ministry of Community and Social Services Urgent Response Guidelines, to support collaboration among developmental service providers in Simcoe County and other community service systems to address unmet urgent support needs.
STATEMENT OF MAJOR RESPONSIBILITIES:
- Provide interventions and supports in urgent situations to prevent further escalation of risk, to mitigate any untenable risks and to address unmet urgent need(s)
- Provide temporary, short-term response or bridging supports until a longer-term support plan and /or solution for the individual can be developed. (0-3months support)
- Collaborative planning among developmental service providers and other community services and resources
- Person-centred approach to planning that promotes least intrusive response to the unmet need.
- Assess individual support needs and risk factors, including health and safety.
- Monitor, evaluate and document individual’s progress while receiving services.
- Advocate and make appropriate referrals, with consent, for supports and services as required.
- Work in collaboration with the Simcoe Resource Management Committee, providing information and updates as required.
- To work as a member of a multi-disciplinary team to support individuals in various settings, including their home and in community.
ORGANIZATION:
- To maintain a co-operative working relationship with professional agencies and community resources.
- To collect and report service delivery data as required.
- To regularly attend and participate in supervision and engage in annual performance appraisals.
- Must be aware of and adhere to Health & Safety Policy Rules & Regulations.
QUALIFICATIONS:
- Post-secondary degree at a university level in one of the social or behavioural disciplines with directly related experience (one to two years) or a community college diploma in one of the social or behavioural disciplines and directly related experience (three to five years) working with individuals with developmental challenges in a community service environment.
- Experience working with persons with developmental delays and mental health issues.
- Ability to show sound clinical judgement with respect to complex client situations.
- Excellent communication and negotiating skills.
- Knowledge of community resources for individuals with developmental needs, proven ability to work effectively on committees, seek out and develop partnership opportunities and formalize service agreements, and relevant legislation.
- Excellent interpersonal and organizational skills showed by the ability to self-manage time and multiple tasks.
- Knowledge of technological applications
- A working understanding of French Language is beneficial.
VALID DRIVER’S LICENCE, ACCESS TO A RELIABLE VEHICLE.
This position is within the Bargaining Unit

We are a team of approximately 100 staff members and believe that collaboration with those we serve, community agencies and government organizations leads to the best outcomes for all.
We provide services within the Ministry of Children, Community and Social Services’ Central and East Region.
