Support Worker , Empower Simcoe, Orillia and Barrie

MINIMUM QUALIFICATION CRITERIA:
- Successful completion of grade 12 high school diploma
- Certificate, diploma and/or degree from an accredited educational institution in Human Services field such as PSW, DSW, Social Worker, ECE, CYW
- Other post-secondary credentials will be considered for equivalency
REPORTING RELATIONSHIP:
Supports and Services Supervisor
PURPOSE OF POSITION:
To provide supportive services to meet the needs of the people in accordance with the policies and procedures of Empower Simcoe and legislative requirements.
The job duties listed hereafter are an overview of the minimum requirements of the position and do not include all of the duties inherent, included or associated with the job or with the performance of the job.
DUTIES AND RESPONSIBILITIES:
Support Workers will be responsible for, but not limited to the following:
- Using a Personal Outcome Measures lens, act as the support person for people residing in the home
- Supporting people to develop and realize individualized goals through the Individual Support Planning process in accordance with the desire of the person and in conjunction with the Supports and Services Supervisor and the Senior Support Worker
- Knowing, contributing to and implementing each person’s service plans
- Completing all required documentation in a timely and effective manner, as outlined in Documentation Due reports and as requested by the Supports and Services Supervisor
- Maximize available resources for people; may include but is not limited to community partnerships, friendships, specialized support
- Acting as an appropriate role model for people
- Providing guidance and direct support in areas identified as necessary, related to activities of daily living, personal care, communication, emotional, physical and mental health, safety, finances, relationship development in an effort to foster independence
- Supporting people to attend community appointments, where deemed necessary; encouraging and supporting people in becoming active members of their community and assisting them with exercising full rights of citizenship
- Facilitating family involvement where appropriate and when requested by the person;
- Working in conjunction with the Senior Support Worker to provide crisis intervention and assistance in accessing community supports as appropriate
- Monitoring person’s behavior, psychoactive medication usage and provide feedback
- Being accountable and responsible for and providing required documentation to verify accountability regarding Agency and the individual’s funds and ensuring timely reconciliations on all purchasing
- Assisting with ensuring successful placements for volunteers and Students as required
- Connecting people supported to resources & programs in the community based on their interests
- Representing the Agency in an appropriate manner in the community by modelling and fostering positive customer service in all aspects of responsibilities
- Ensuring proper use of and maintenance of Agency equipment, buildings, property and other assets
- Ensuring that the work place is maintained in a clean, safe and secure manner
- Performing their duties in a safe and responsible manner
- Protecting their own health and safety by complying with the Occupational Health and Safety Act and company’s safe work practices and procedures and perform their duties in a safe and responsible manner
- Demonstrating understanding of and compliance with legislative knowledge of the Employment Standards Act, Human Rights Act, Collective Agreement (if applicable) and other Acts and Legislations as it applies to the role
- Performing other duties or special assignments as assigned.
- Actively participating and supporting the person to work toward achieving their personal outcomes
- Ensuring all concerns related to supports is reported in a timely manner to the Supports & Services Supervisors/ Senior Support Worker
